Heartland Human Care Services Data Use Disclosure for Participants
Below you will find information explaining how participant information is stored and used in Heartland Human Care Services’ (HHCS) Salesforce data system:
- Heartland Human Care Services (HHCS) collects personal information from participants so we can ensure you are receiving the most relevant services.
- HHCS may use your de-identified data to examine our impact and/or ways to improve our programming (De-identified data is information that cannot be used to identify you)
- All personal information is private.
- HHCS uses a system called Salesforce to keep your information. Salesforce makes sure your information is private through the use of:
- Encryption (this means data is coded so it can’t be shared if hacked)
- Firewall (another way to protect information)
- Password-protection
- Controlled access to data (information is only available to necessary HHCS staff)
The ONLY time a small amount of your information might be shared is if:
- We talk to staff or another service provider to connect you to more services
- A funder asks for information. If this happens, personal information like name and address is removed.
- We contact you to ask if you can complete a program satisfaction survey
- We receive a court order and have to give testimony or records for a court case
- If you are an immediate threat to yourself or others
