Participants who are homeless at entry and who also have at least one disability, are placed into privately owned, scattered-site apartments in the city of Chicago with independent landlords. The program pays a rental subsidy directly to the landlord. Participants are provided with intensive case management and referrals to other services such as counseling, employment, education, and legal. The program helps participants to maintain housing stability, maintain or increase income, employment and/or benefits, and to maintain or improve physical/mental health outcomes.
Applicants come from the City of Chicago’s Coordinated Entry System (CES) or Housing Opportunities for People with HIV/AIDS (HOPWA) program. For CES, please visit one of the in-person access points to get on the list. For HOPWA (specific to people living with HIV), speak with your case manager or contact the AIDS Foundation of Chicago at 312-922-2322 or online.
